RFQ 75H71325Q00011 is hereby amended to add Clause 52.237-7 Indemnification and Medical Liability Insurance and ATTACHMENT 6 – QA. The deadline for the receipt of quotes is hereby extended by 7 days to January 30, 2024.
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71325Q00011 has been issued as a Request For Quotation for a firm fixed price commercial services contract for Behavioral Health Nonpersonal Services. See the attached Performance Work Statement for a description of the services to be provided. The performance period is one year, to start within 30 days of the date of award. The performance period includes a one-year base period, and four (4) one-year option periods. The place of performance is Western Oregon Service Unit (Chemawa Indian Health Center), 3750 Chemawa Road NE, Salem, Oregon 97305.
