INTRODUCTION

This project intends to obtain a qualified Subcontractor to perform interior repairs including bathroom demolition, bathroom reinstall, replacement of galvanized pipe, painting of kitchen cabinets, replacement of countertops, interior painting, installation of light fixtures, and replacement of flooring at Village House Potawatomi 14 at Fermi National Accelerator Laboratory (Fermilab) located in Batavia, Illinois. Fermilab is operated by the Fermi Research Alliance (FRA).  FRA will provide a Task Manager / Construction Coordinator (TM/CC) to provide oversight of the project.

The purpose of this statement of work (SOW) is to obtain the supervision, expertise, labor, materials, and equipment in relation to the work indicated below.

SCOPE OF WORK

The Subcontractor shall perform the following:

Bathroom Demolition

1. Mobilize to Potawatomi 14 in the Fermilab Village.
2. Cover door openings with plastic sheeting (or similar) to prevent the spread of dust to other areas.
3. Ensure that water to the bathroom is secured, as necessary.
4. Remove all currently installed fixtures, flooring, wall tile, and cabinets from the bathroom.
5. Remove all existing galvanized pipe.
6. Dispose of removed items and debris in dumpster provided by FRA.
7. Perform close out of job site.
   1. Ensure all tools, equipment, and waste are removed.
   2. Perform turnover / closeout of job with TM/CC.

Bathroom Remodel

1. Mobilize to Potawatomi 14 in the Fermilab Village.
2. Replace all existing galvanized pipe in the bathroom with copper pipe.
3. Adapt and renovate any plumbing positions as necessary for installation of new fixtures.
4. Determine if any changes are required for electrical outlets or fixtures.
5. Perform Lock Out / Tag Out (LOTO) of any electrical outlets or fixtures as needed.
6. Install new light fixture, fan, and outlets equivalent to what is currently installed in the bathroom.
7. Install new vinyl wall and floor tile equivalent to that currently installed.
8. Install new single sink bathroom vanity (grey cabinet with white vanity sink) and verify leak tight.
9. Install new standard acrylic white bath tub / shower surround and verify leak tight.
10. Install new standard white, round toilet and verify leak tight.
11. Install standard satin nickel finish towel bar, paper holder, and shower curtain bar.
12. Dispose of any garbage / debris in dumpster provided by FRA.
13. Perform close out of job site.
    1. Ensure all tools, equipment, and waste are removed.
    2. Perform turnover / closeout of job with TM/CC.

Kitchen Countertop Demolition

1. Mobilize to Potawatomi 14 in the Fermilab Village.
2. Ensure that water to the kitchen is secured, as necessary.
3. Disconnect sink drains and water lines from faucet and ensure there is no connection to countertop.
4. Remove kitchen countertop taking care to preserve kitchen cabinets.
5. Clean any remaining adhesive from cabinets.
6. Dispose of old countertop in FRA provided dumpster.

Paint Kitchen Cabinets

1. Mobilize to Potawatomi 14 in the Fermilab Village.
2. Remove all hardware and doors from kitchen cabinets.
3. Prep and clean cabinets for painting by using a degreaser and lightly sanding, as required.
4. Prime cabinets to ensure paint adheres to the surface.
5. Paint the cabinets using a minimum of two coats Benjamin Moore EcoSpec in color #1600 Timber Wolf.
6. Reinstall doors and hardware.
7. Perform close out of job site.
   1. Ensure all tools, equipment, and waste are removed.
   2. Perform turnover / closeout of job with TM/CC.

Install Kitchen Countertop

1. Mobilize to Potawatomi 14 in the Fermilab Village.
2. Measure the installation area for the countertop to ensure proper fit.
3. Cabinets should be laminate in Wilsonart White Carrara 4924-38 or equivalent.
4. Dry fit the countertop and scribe to fit any irregularities in the wall or cabinets.
5. Cut or sand along scribe line, as necessary, to ensure proper fit.
6. Fasten countertops to cabinets.
7. Caulk any seams or gaps.
8. Reinstall sink and caulk any seams or gaps.
9. Reattach sink drains and faucet.
10. Perform close out of job site.
    1. Ensure all tools, equipment, and waste are removed.
    2. Perform turnover / closeout of job with TM/CC.

Carpet Removal and Installation
